*** If you do not receive an acknowledgement that your online form was received and we are working on it within 2 hours of sending and it is between 7 a.m. – 7 p.m. when you submitted the form, please contact us immediately.

*** If you submitted the Request Form after normal business hours, it will be handled the next business day.  Late night submission for next morning appearances may not be able to be covered.
